We are delighted to announce that patients of our practice have started to be invited for the Covid-19 vaccination. The programme rollout is phased to ensure vaccine deliveries, logistics and staffing are robustly implemented and consequently, in line with national guidance, we are only inviting those at high clinical risk over the age of 80 to come for vaccines first. As you can appreciate, this first cohort of patients is a lot of people in our local community that we need to contact. Please be assured that we are working tirelessly to telephone patients who are eligible to make vaccination appointments as soon as they are available.


 


We ask all of you in our practice community to please be patient and wait for your call. Our phones are incredibly busy with poorly patients seeking medical advice and treatment at this time of year and we must ask that you do not call us to enquire about the Covid-19 vaccination. As more vaccine is delivered, more people from other eligible cohorts will be contacted and invited for vaccination in due course. You may print off a registration form, fill it out and bring it in with you on your first visit to the practice.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. There is an online version of this file too, which you may fill out and send to us. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.
